Intake Coordinator
Duties:
-Provide intake oversight and assess clinical documentation to ensure care is provided
- Coordinate and facilitate the intake process for new patients
- Upload discharge paperwork to EMR
- Maintain accurate and up-to-date patient records
Responsibilities:
- Assist the agency with completing each online intake and providing accurate information
- Assist in forming intake processes and procedures
- Answer phones, receive intake information, and enter information into computer system.
- Access multiple websites to access patient information for discharge paperwork.
- Facilitate a smooth transition from hospital to home by documenting all relevant and case specific data in the EMR
- Work closely with discharge case managers and develop positive professional relationships with them.
- Communication skills are a must have
- Actively participate and contribute to team meetings.
Job Conditions:
- The ability to communicate well, verbally and in writing
- The ability to assess the clinical nature of each intake and provide input on the patient car that will be needed.
Equipment operation:
- Utilization of calendar, multi line telephone system, computer, EMR, fax machine and copier.
Qualifications:
- Previous experience in a healthcare or clinical setting preferred( One or more years in a home health setting or hospital)
- Strong organizational and communication skills
- Attention to detail and ability to maintain confidentiality
- Must pass criminal background check
- Must be licensed to operate a motor vehicle
