A Day in the Life of an ADvantage Home Care RN Case Manager
As an ADvantage Home Care RN Case Manager, you play a key role in ensuring members receive the services they need to remain safely in their homes. Your responsibilities include:
- Arranging and coordinating service plans
- Providing nursing assessments and education
- Collaborating with case managers and other care team members
- Ensuring documentation meets agency and ADvantage program standards
- Supporting members and families with professionalism, compassion, and respect
- Maintaining regular attendance and punctuality
- Home visits with members
Qualifications
- Active RN license in the state of Oklahoma
- No previous ADvantage experience required.
- CPR certification within 60 days of hire
- Valid driver's license and good driving record
- Reliable transportation with proof of insurance
- Ability to travel long distances and adapt to frequent schedule changes
