Responsibilities:
- Work closely with nurses, social workers, chaplains, physicians, and volunteers to provide high-quality hospice care.
- Provide high-quality personal care to patients facing a terminal illness.
- Follow through with the comprehensive nursing care plan as instructed by the RN.
- Protect and respect patient and caregiver rights and privacy at all times.
- Display a pleasant, courteous, and professional attitude toward clients, the general public, and nursing home staff.
- Make visits wherever the patient resides, including private homes, skilled nursing facilities, assisted living facilities, and hospitals.
- Document all visits in the electronic medical record in a timely fashion.
- Sit with patients for caregiver relief and through the last hours of life.
- Current, active CNA license in the state of Missouri.
- Computer and smart phone skills for EMR charting, email, apps for mileage computation, etc.
- Hourly rate plus mileage reimbursement.
- Work hours are primarily Monday through Friday, 8am to 5pm, with some after-hours needs from time to time.
- Flexibility with scheduling and hours is a must.
