- Conducting assessments and developing individualized care and rehab plans
- Administering medications and treatments appropriately
- Coordinating with therapy and interdisciplinary teams to ensure safe recovery and optimal outcomes
- Educating residents and their families about care plans
- Accurately documenting in the EMR system
- Supporting CNAs and other staff members
- Upholding the organization’s mission of dignity, respect, and person-centered care
- Current licensure as a Registered Nurse
- Experience in rehab or senior care settings preferred
- Strong communication and teamwork skills
- Ability to provide compassionate, person-centered care in a collaborative environment
