Physical Therapist Role Summary
- Provide one-on-one physical therapy to older adults in private residences and senior living communities.
- Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments aimed at improving mobility, independence, and quality of life.
- Document patient progress accurately in an electronic medical record system, ensuring regulatory compliance.
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff on safety, function, and aging-in-place strategies.
- Utilize telehealth and virtual care options to extend access and flexibility when appropriate.
- Valid Physical Therapy license or eligibility in practice state(s).
- Degree from an accredited Physical Therapy program.
- Proficiency in computers and EMR systems.
- Current CPR certification.
- Ability to exert medium physical effort, including lifting up to 50 lbs occasionally.
- Perform patient transfers, repositioning, and therapeutic activities.
- Drive to patient locations, stand/walk for extended periods, and wear PPE as needed.
