Certified Nurse Practitioner (NP) / Physician Assistant (PA)— Pulmonary Specialty | Long-Term Care (Pulmonary)
Preferred Healthcare provides high-quality, compassionate pulmonary specialty care to residents in long-term and post-acute care facilities across the Greater Columbus area.
We’re hiring a Certified Nurse Practitioner or Physician Assistant to round in skilled nursing/long-term care settings and manage a geriatric population with complex, chronic pulmonary conditions. Training and ongoing mentorship are provided by a Board-Certified Pulmonologist.
What You’ll Do
- Provide pulmonary-focused assessment, diagnosis, and management for LTC/post-acute residents
- Round in assigned facilities and deliver consistent, high-touch follow-up care
- Collaborate with nursing, therapy, and facility leadership to coordinate care plans
- Document efficiently and accurately in the EHR; follow evidence-based standards
- Build strong relationships with residents, families, and facility staff
What You Bring
- Active Ohio NP license (required)
- 1 year experience in geriatrics, LTC/SNF, or chronic disease management (preferred)
- Strong clinical judgment, assessment skills, and comfort managing complex patients
- EHR proficiency and strong/timely digital documentation habits
- Clear communication, professionalism, and a team-first mindset
