Key Responsibilities:

• Schedule and coordinate home healthcare visits for hospice patients, including home care visits, admissions, and discharges
• Ensure timely and effective communication with patients, families, and healthcare providers
• Maintain accurate and up-to-date records of patient visits, medication administration, and care plans
• Monitor and manage daily schedules, including tracking staff availability and coverage
• Collaborate with interdisciplinary team members to develop and implement patient care plans
• Provide exceptional customer service to patients, families, and referral sources
• Maintain compliance with regulatory requirements and company policies
  
  
  

Requirements:

• 1 years of experience as an HHA Schedular or related field
• Ability to work in a fast-paced environment and prioritize multiple tasks
• Excellent organizational and communication skills
• Proficiency in electronic medical records and scheduling software
• Ability to maintain confidentiality and handle sensitive information
