Position Summary
The Community Outreach & Referral Coordinator will be responsible for developing and maintaining relationships with referral sources and community organizations. The ideal candidate is comfortable making phone calls, sending emails, attending community events, visiting organizations, and following up consistently with potential referral partners.
This position includes a competitive hourly rate of $15–$17/hour plus performance-based bonuses for meeting established outreach and organizational goals.
Key Responsibilities
- Identify and develop relationships with potential referral sources and community partners
- Conduct professional outreach through phone calls, emails, visits, presentations, and networking
- Build relationships with healthcare providers, therapists, physicians, schools, social service organizations, community organizations, and other appropriate referral partners
- Follow up consistently with referral sources and community contacts
- Maintain accurate records of outreach activities, contacts, meetings, and opportunities
- Schedule and coordinate meetings, presentations, and community outreach activities
- Represent the organization professionally at community events and networking opportunities
- Educate community partners about the behavioral health services offered by the organization
- Identify opportunities to increase awareness of available services
- Communicate referral opportunities to the appropriate internal team members
- Assist with tracking referral activity and outreach performance
- Meet established weekly and monthly outreach goals
- Provide regular reports regarding outreach activities, relationship development, and referral opportunities
- Maintain professionalism and confidentiality when discussing clients and services
- Perform other duties related to community outreach and business development as assigned
Compensation
$15.00–$17.00 per hour performance-based bonuses
Our bonus program is designed to reward employees who consistently meet or exceed established outreach, relationship-building, and organizational performance goals.
Additional earning potential is available based on performance.

Preferred Qualifications
- Previous experience in healthcare, behavioral health, social services, community outreach, marketing, sales, business development, or referral coordination
- Experience developing professional relationships with healthcare or community organizations
- Knowledge of behavioral health services is a plus
- Experience working with referral sources is preferred
Education: High school diploma or equivalent required. College coursework or degree in business, marketing, healthcare administration, social services, psychology, human services, or a related field is a plus.
Performance Expectations
During the first 90 days, the selected candidate will be expected to establish a consistent outreach routine, develop new community relationships, maintain regular follow-up with referral sources, and contribute to measurable growth in referral opportunities.
