HOME HEALTH PHYSICAL THERAPISTJob Title: Physical Therapist – Home HealthRate: Up to $2300.00 weeklyLocation: Agawam, MADay (8H) - (5x8hr) - (8A - 5P)Key ResponsibilitiesPatient Assessment: Evaluate patients' physical function and limitations within their home environment to develop individualized treatment plans.Treatment Implementation: Provide therapeutic interventions aimed at improving mobility, strength, balance, and pain management.Education: Educate patients and caregivers on diagnoses, safety precautions, equipment use, and home exercise programs.Collaboration: Work with an interdisciplinary team, including nurses and occupational therapists, to create comprehensive care plans that address the patient's physical, emotional, and social needs.Documentation: Maintain accurate records of patient evaluations, treatment plans, and progress according to healthcare standards.EducationRequired a degree in physical therapy (Bachelor's or Master's)ExperiencePrevious experience in home health care of similar setting is preferred.Licenses/CertificationsMust hold a valid state license to practice physical therapyMust be able to obtain and maintain CPR certification.
