Licensed Practical Nurse (LPN) – Permanent

Position Details
- Position: Licensed Practical Nurse (LPN)
- Employment Type: Permanent
- Location: Berlin, NH
- Shift: Nights (7:00 PM – 7:00 AM)
- Schedule: Three 12-hour shifts per week
- Every other weekend rotation
- Union position
Requirements
- Graduate of an accredited Practical Nursing program
- Current New Hampshire LPN License
- Current AHA BLS Certification
- Long-term care or skilled nursing experience preferred
Key Responsibilities
- Provide skilled nursing care in accordance with physician orders, resident care plans, and established standards of care.
- Assist with assessing, planning, implementing, and evaluating resident care needs.
- Administer medications, treatments, and nursing interventions as ordered.
- Monitor resident conditions and communicate changes appropriately to physicians and nursing leadership.
- Supervise and support LNAs/LMNAs during assigned shifts.
- Assist with resident admissions, transfers, discharges, and care coordination as needed.
- Collaborate with physicians, residents, families, and the interdisciplinary care team.
- Complete accurate and timely clinical documentation.
- Ensure compliance with state regulations, facility policies, and professional nursing standards.
