We are seeking a dedicated and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team. The ideal candidate will provide comprehensive mental health care to patients in a geriatric unit, conduct assessments, complete patient rounding's, and develop treatment plans. This role requires a strong understanding of psychiatric conditions and the ability to work collaboratively within a multidisciplinary team to enhance patient outcomes.

Duties

• Conduct thorough diagnostic evaluations and assessments of patients with mental health disorders.
• Complete initial evaluations, follow up evaluations, and the discharge of patients
• Develop and implement individualized treatment plans based on patient needs and preferences
• Provide health coaching to patients, guiding them in managing their mental health conditions effectively.
• Maintain accurate and timely documentation of patient interactions and treatment progress.
• Collaborate with other healthcare professionals to coordinate care for patients.
• Excellent communication skills with the ability to build rapport with patients and families.

Qualifications

• Master’s degree in nursing with a specialization in Psychiatric Mental Health Nurse Practitioner from an accredited program.
• Current PMHNP certification and active California state licensure as a Nurse Practitioner.
• Active CA DEA
