Primary Responsibilities:
Assist with transfers, positioning, and wheelchair mobility.
Provide sponge baths, personal hygiene, grooming, and dressing assistance.
Assist with diaper changes and toileting needs.
Prepare and provide meals and assist with feeding as needed.
Maintain a clean, safe, and comfortable environment.
Follow the individual’s care plan, routines, and health/safety instructions.
Monitor and report changes in the individual’s condition, behavior, or needs.
Provide companionship and support with daily activities.
Complete required daily documentation and service notes.
Maintain the individual’s dignity, privacy, and independence at all times.
