We are currently seeking a PART-TIME Therapy Clinical Services Manager for our Northern Home Health Therapy Division.
This is a part-time position located in our Appleton, Fond du Lac, or Sheboygan office.

As the Regional Therapy Manager, you will assume full responsibility for the review of the Plan of Care for your home health patients making necessary changes to maintain organization, state, and federal compliance. This includes:

• The timely processing of day-to-day workflow tasks following knowledge of CMS and value-based purchasing guidelines.

• Review of incoming referrals to determine home care eligibility.

• Working with the therapy scheduler on ensuring successful hand off services.

• Hosting case conferences for our northern counties

• Leading discipline-specific meetings

• Conducting annual reviews, competency training, onsite audits, and triaging cases with the active therapy team.

Other responsibilities include, but are not limited to:

• Processing orders.

• Analyzing data and clinical metrics.

• Assisting with DME handling.

• Chart audits.

• Intake obligations.

• Working with the quality team.

The manager will have administrator on call requirements approximately once every 8-10 weeks.


To be successful in this role, you will need the following:

• Sound therapy skills and experience.

• At least one year of experience in home care.

• Active credentials in the state of Wisconsin.

• Ability to work independently.

• Excellent written and oral communication.

• Exceptional time management skills.
