We are seeking a Full-Time Home Health LPN Visit Nurse to provide care in
Fond du Lac and Dodge Counties

As the LPN Visit Nurse, you will provide quality care to our home health patients, working closely with the RN Case Manager. Duties include, but are not limited to:

• Performing routine nursing visits

• Providing wound care -- we take great pride in being a preferred provider for several local wound care clinics

• Providing catheter care

• Excellent communication skills with clients, families, doctors, facility staff, and co-workers is a must to be successful in this role

• Ability to work independently

• Good organization and time management skills are also critical

• Love working with the geriatric population

Experience Requirements:

• One year of clinical experience preferred.

License Requirements:

• Current Wisconsin LPN licensure

• Current driver's license, valid vehicle insurance, and access to a dependable vehicle

Position Details:

• Location: Brown, Winnebago, and Calumet Counties

• Schedule: Full-Time, 1st shift Monday-Friday. Minimal on-call required.

• Compensation: $25-$30/hour based on experience.
