We are currently seeking a Licensed Practical Nurse who would love to work in our busy Home Health Intake Department.

This is a full-time position working from our Germantown office.

As a Home Health Intake Coordinator, you will assume full responsibility for coordinating the smooth transition of patients onto our services and for communicating with our care team to make any necessary changes as the need arises. This includes:
- Establishing and maintaining positive working relationships with current and potential referral sources.
- Building and monitoring community and customer perceptions of Preceptor Home Health & Hospice as a high-quality provider of services.
- Maintaining comprehensive working knowledge of Medicare guidelines and ensures that patients are admitted according to contract provisions.
- Taking responsibility for obtaining all necessary paperwork and orders for processing referral.
- Entering referrals into EMR system to include but not limited to medication lists, insurance checks & verifications, and uploading all patient information received.
Experience Preferences:
- One year of healthcare experience
License Requirements:
- Must have a current LPN license in the state of WI.
Position Details:
- Schedule: Full-Time, 7:30 AM to 4:00 PM Monday-Friday.
- Compensation: $24-$28 per hour; negotiable based on experience.
