Responsibilities And Accountabilities
- Assessment and Planning: Conduct thorough assessments, maintain the care plan, and lead groups to enhance patient understanding and engagement.
- Patient and Family Engagement: Maintain open lines of communication with patients and their support systems to involve them in treatment plan decisions and foster a collaborative approach to care.
- Ethical Advocacy: Deliver and advocate for patient-centered, culturally appropriate, and age-appropriate care, ensuring that all services align with ethical standards and best practices.
What Makes You a Great Fit
- Master’s Degree in Social Work with licensure in the State of Minnesota required.
- Experience in healthcare setting preferred
- Experience using electronic health records preferred
- Proficient in Microsoft Office Products including Word and Outlook
- Able to effectively and efficiently communicate in written and verbal form
- Ability to smoothly and rapidly shift between dissimilar tasks
- Ability to communicate well both verbally and in writing
- Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
- Ability to make effective decisions while in stressful situations and circumstances
Salary : $26 - $34
