Job Summary
This part-time (0.7 status or 28 hours weekly) position provides nursing care in the home care setting in accordance with established standards, policies, procedures, licensure, and scope of practice. Home Health and Hospice care is provided for patients ranging from infant through geriatric, including terminal states.
Wage range: $32.00-$49.46
Responsibilities include:
- Utilizes the nursing process to provide care in the home setting with documentation to show assessment and intervention of the home care visit.
- Utilizes independent nursing judgment to coordinate the patient care and OASIS assessment process.
- Completes admission interviews and assessments and establishes patient-driven priorities to include the interdisciplinary team approach for the home care and hospice plan of care. Includes the nursing home staff, assisted living staff, and family support structure to realize successful outcomes.
- Administers prescribed medications, treatments, and patient-specific cares to patients in accordance with approved nursing techniques.
- Facilitates and coordinates patient care via the interdisciplinary team approach from admission to discharge. Assumes the team leader/facilitator role in collaborating with therapy staff, nursing home staff, assisted living staff, and family.
- Assesses patient/family records significant conditions and reactions and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents throughout the progression of care.
- Follows medical treatment plan through execution of physician orders.
- Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disability/illness.
- Supervises and trains LPN's, aides, and volunteers in the processes of patient care.
- Other duties may be assigned.

Qualifications
Graduate of an approved Registered Nurse Program. Two years’ experience in Medical/Surgical nursing required.
Holds or is eligible for current RN licensure in the State of South Dakota.
Valid South Dakota driver’s license required.
Mandatory Basic and Advanced Life Support Training per policy.
