RN Case Manager – Hospice & Home Care
Reports To: Clinical Director
Status: Full-Time (Approximately 40 hours/week)

Prairie Home Hospice and Community Care is seeking a compassionate RN Case Manager who wants to make a profound difference in the lives of patients facing serious or terminal illness. In this role, you are more than a clinician — you are an advocate, educator, leader, and steady source of comfort.
What You’ll Do
As the primary nurse for your patients, you will:
- Lead and coordinate individualized plans of care
- Provide skilled nursing care and expert symptom management
- Support patients and families through education, compassion, and clear communication
- Collaborate closely with physicians and an interdisciplinary care team
- Supervise and mentor care team members
- Ensure care is delivered with excellence, integrity, and dignity
You will be present for difficult conversations. You will help manage pain and ease anxiety. You will guide families through uncertainty with confidence and empathy. Your clinical skill will matter — but so will your heart.
What We’re Looking For
- Active RN license in good standing
- Experience in hospice, home health, or related field preferred
- Strong clinical judgment and critical thinking
- Ability to work independently while thriving in a collaborative team
