You’ll have the support of physicians, care managers, social workers, and coordinators—allowing you to focus on delivering thoughtful, high-quality care.
What You’ll Do
- Deliver in-home primary care for medically complex geriatric patients
- Manage chronic and acute conditions and develop care plans
- Conduct comprehensive assessments and follow-up visits
- Collaborate with physicians and interdisciplinary team members
- Educate and support patients and families
- Licensed Nurse Practitioner in New Jersey
- Board Certified (AANP or ANCC)
- Primary care, internal medicine, or geriatrics experience preferred
- Comfortable working independently in a home-based setting
- Valid driver’s license
- Physician-led organization with 30 years of experience
- National leader in home-based primary care
- Established, high-functioning, collaborative NJ team
