Job Title: Registered Nurse - Case Manager
Location: Buckley AFB Health Clinic, Colorado
Reports To: TBD
Schedule: Monday - Friday, 0730 to 1630 (9-hour shifts with a 60-minute unpaid lunch)

The Registered Nurse - Case Manager (RN-CM) provides comprehensive case management services to patients at Buckley AFB Health Clinic. The RN-CM plays a critical role in coordinating patient care across the healthcare continuum, ensuring that services are efficient, high-quality, and tailored to meet individual needs. This position requires a thorough understanding of nursing, case management principles, and healthcare systems.

• Education: Associate Degree in Nursing from a program accredited by ACEN or CCNE.

• Licensure/Certifications: Must possess one of the following case management certifications:

  • CCM, CDMS, CRRN, COHN, COHN-S, ACCC, CRC, RN-NCM, or CMC.

• Experience: Minimum of one year of professional nursing experience.

• Manage all phases of the Case Management Program (CMP) in alignment with established CM standards of care.

• Deliver expert nursing input in care assessment, planning, implementation, coordination, and monitoring.

• Identify and act on case management opportunities in both inpatient and outpatient settings, including telehealth.

• Develop tools and strategies to support case management such as patient assessments, clinical guidelines, and resource databases.

• Integrate nursing case management with utilization management (UM) and social work services.

• Establish and maintain relationships with community agencies and organizations to support patient care.

• Document patient care data accurately and in a timely manner.

• Develop and coordinate individualized treatment plans that include preventive, therapeutic, rehabilitative, and psychosocial interventions.

• Ensure continuity of care through appropriate discharge planning and follow-up.

• Educate patients and caregivers based on identified healthcare needs.
