Responsibilities
- Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Provide patient observation and monitor vital signs to ensure health and safety; recognize and respond promptly to changes in condition
- Support medication administration following HIPAA guidelines and care plans developed by healthcare professionals
- Prepare nutritious meals in accordance with dietary restrictions and assist residents during meal times to promote independence
- Maintain a clean and safe environment through regular cleaning duties, including laundry, sanitation of living areas, and adhering to infection control protocols
- Document care activities accurately in compliance with HIPAA regulations; update progress notes and incident reports as needed
- Foster social engagement by encouraging participation in group activities and community outings; support residents with Alzheimer’s, dementia, or other memory care needs through specialized techniques
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated knowledge of patient care techniques including assisting with ADLs, medication administration, and patient monitoring
- Experience working with individuals with developmental disabilities, autism spectrum disorder (ASD), or memory care needs such as Alzheimer’s or dementia is advantageous
- Certification in CPR and First Aid is required; additional certifications like HIPAA training or hospice care experience are a plus
- Ability to perform heavy lifting safely and operate equipment such as Hoyer lifts; physical stamina for long shifts is essential
- Strong understanding of home & community care practices along with familiarity with long-term care standards and social work principles
