What are the responsibilities and job description for the Substance Use Counselor position at POSTGRADUATE CENTER FOR MENTAL HEAL?
JOB SCOPE:
This position requires in-depth knowledge of substance use disorders, substance use treatment including medication assisted treatment, harm-reduction, stages of changes and other evidence-base practices. Conduct thorough evaluations, on-going risk assessments, manage risk, develop treatment plans and provide evidence-based counseling to an ethnically and culturally diverse population of children, adolescents, adults, and families. Responsible for documenting and maintaining all treatment and services in the clinical case record. Collaborate with otherstaff and external service providers around the provision of additional services, including psychiatric rehabilitation, substance use, medication management, physical healthcare, and peer support.
ESSENTIAL FUNCTIONS:
- Provide direct substance use services to a caseload of individuals and families.
- Conduct group and individual sessions in the clinic setting and in the community.
- Provide crisis assessment, crisis planning, and crisis intervention services.
- Complete comprehensive intake assessments, ongoing screening, assessment and diagnostic evaluations for individuals identified with potential substance use disorders.
- Provide person- and family- centered services and treatment planning.
- Proving ongoing substance use counseling and group services, utilizing trauma informed care, motivational interviewing, harm-reduction and other evidence-based practices.
- Provide consultation for clients who may need services or in need of short-term clinical interventions.
- Facilitate the collection of toxicology screens and review results with individuals.
- Document all services and outreach efforts via progress notes in the EHR within 24hours.
- Collaborate with medical and other staff regarding the coordination of care and services.
- Collaborate with external treatment providers to coordinate care.
- Perform other duties, consistent with the goals and objectives of the program, as directed by supervisor.
Qualifications:
EDUCATION AND EXPERIENCE:
- A minimum of a CASAC – T is required.
- Full CASAC or advanced is preferred.
- Experience working with mental health and substance use disorder populations.
- Experience in work with families and groups.
- Must be outcome oriented in a fast-paced environment with deadlines.
Salary : $55,000 - $65,000
