Speech-Language Pathologist (SLP) – Full-Time

Post Rehab is seeking a Speech-Language Pathologist (SLP) to join the rehabilitation team at Epsom Health Care Center. This full-time opportunity offers the chance to work closely with residents on communication, cognitive, and swallowing needs while being part of a supportive skilled nursing and post-acute care team.
What You'll Do
- Evaluate residents with speech, language, cognitive-communication, and swallowing disorders
- Develop individualized treatment plans based on clinical findings and resident goals
- Provide skilled interventions focused on communication, cognition, and dysphagia
- Monitor resident progress and adjust treatment approaches when appropriate
- Educate residents, families, and caregivers on therapeutic strategies and recommendations
- Complete accurate and timely clinical documentation
- Collaborate with PT, OT, nursing, physicians, and other members of the interdisciplinary care team

Qualifications
- Active New Hampshire Speech-Language Pathologist license required
- Graduate degree in Speech-Language Pathology from an accredited program
- Strong clinical, communication, and organizational skills
- Skilled nursing or rehabilitation experience preferred, but not required
