A skilled nursing and rehabilitation facility in Illinois is seeking an organized, personable, and motivated Admissions Coordinator to manage the admissions process and help prospective residents and their families navigate their transition into care.

Responsibilities

• Coordinate admissions from initial inquiry through admission.

• Communicate with prospective residents and families regarding services and care options.

• Conduct facility tours and answer questions about programs, amenities, and policies.

• Collect medical records, admission paperwork, and required documentation.

• Coordinate with nursing, social services, and other departments to facilitate timely admissions.

• Maintain accurate admissions records and ensure documentation is completed.

• Follow up with residents and families to support a positive admissions experience.

Qualifications

• Bachelor's degree in healthcare administration, social work, or a related field preferred.

• Experience in healthcare admissions, skilled nursing, rehabilitation, or a related setting preferred.

• Excellent communication, organizational, and interpersonal skills.

• Ability to manage multiple priorities and work collaboratively with healthcare professionals and families.

• Knowledge of post-acute care and the local healthcare market preferred.
