Advanced Practice Provider (NP or PA) – Post-Acute Care / Skilled Nursing
Location: Jacksonville, NC and surrounding area
Schedule: Weekdays Only – No Nights, Weekends, or On-Call afterhours/weekends/Holidays
About the Role
We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our post-acute care team serving skilled nursing facilities (SNFs) and long-term care (LTC) patients. In this role, you will work in direct supervision with two established post-acute care physicians who have over 30 years of combined experience in this setting, providing patient-centered medical management for individuals recovering after hospitalization or needing long-term care services.
What You’ll Do
- Perform new patient admissions, histories, and physical exams.
- Manage acute care and chronic medical conditions in collaboration with physicians.
- Order and interpret lab work, imaging, and other diagnostics; prescribe medications per licensure.
- Provide wound care, pain management, palliative care, and preventive health education.
- Collaborate with nursing staff, therapy teams, and families to ensure safe care transitions.
- Maintain accurate and timely documentation in the electronic health record (EHR).

Qualifications
- Current state license as a Nurse Practitioner or Physician Assistant.
- National certification (Adult, Family, Acute Care, or Geriatrics preferred).
- DEA registration (if applicable).
- 1–2 years’ experience in post-acute care, skilled nursing, hospitalist medicine, geriatrics, or rehabilitation preferred.
- Strong clinical assessment, communication, and teamwork skills.
Work Environment
- Practice in skilled nursing facilities and long-term care settings.
- Regional travel between facilities may be required.
