Job summary

This position is responsible for implementing the formal and informal, strength based guidelines of family assistance into the community to ensure that professional services are provided in an effective manner. This position requires a good understanding of Positive Spin, Inc’s policies and organizational procedures with the ability to work alongside other people in a service-oriented environment. 


Summary of essential job functions

·      Conduct initial intake and assessment of qualifying client families at risk.

·      Assess the risk and initiate/document appropriate paperwork and files that reflect individualism, cultural competence, family empowerment/support, and safety while maintaining accurate records of services provided with actions taken.

·      Facilitate a minimum of two Family Team Conferences a month. (If applicable)

·      Determine the appropriate services to include on the Family’s Individualized Course of Action (ICA).


Coordinate with the Program Supervisor to insure follow-up on tasks relating to the client’s Individual Course of Action Plan.

·      Present cases for supervisory review as directed.

·      Implement prevention focused strategies by helping and advocating for families to gain access to resources. Provide appropriate referrals to resources for client services and conduct formal as well as informal supportive counseling.

·      Be a visible support in the community; collaborate with other social service agencies, and engage families that may not need judicial action to keep them out of the “system”.

·      Follow all procedures and use all skills demonstrated during the Family Support / Individual Course of

Action Plan Certification Training. 

·      Ability to submit assignments, monthly report information, etc. accurately and in a timely manner.

·      Timely and accurate data entry into the client database.

·      Participate in community activities.

·      Comply with flexible work hours as needed.

·      Performs other related duties as assigned.


Skills

·      Must be proficient in MS Word, Excel, Outlook, and Windows

·      Stress and time management skills

·      Ability to work well with families, peers, supervisors, and community


Minimum requirements

·      2 year degree and experience working in human services; 4 year degree preferred

·      3 years experience working with families in child welfare system or families at risk of entering the system is desirable

·      Familiarity with System of Care principles.

·      Certification in Family Team Conferencing within 3 months of hire


Working Hours

Daily M-F from 8:30am to 5:00 pm. Flexibility in work hours is required

(Evenings and /or weekend hours may be required on occasion to support program activities/events)
