Job Summary And Qualifications
The Outpatient Behavioral Health MSW Social Worker is responsible for the provision of psychosocial assessment, individual and family therapy sessions, psychotherapy and psychoeducation groups, and treatment planning in the outpatient programs. As a multidisciplinary team member, you’ll integrate a systems perspective in the development and maintenance of the program milieu and the treatment and discharge planning process. Functions in the role of the case coordinator, streamlining the assessment, treatment and discharge planning processes in a timely, high quality, and cost-efficient manner.

Job Summary And Qualifications
The Outpatient Behavioral Health MSW Social Worker is responsible for the provision of psychosocial assessment, individual and family therapy sessions, psychotherapy and psychoeducation groups, and treatment planning in the outpatient programs. As a multidisciplinary team member, you’ll integrate a systems perspective in the development and maintenance of the program milieu and the treatment and discharge planning process. Functions in the role of the case coordinator, streamlining the assessment, treatment and discharge planning processes in a timely, high quality, and cost-efficient manner.
What will you do in this role:
- Provides psychosocial assessment for all assigned patients within relevant time standard: 72 hrs.
- Coordinates treatment planning and maintains ongoing contact with outpatient providers for continuing care of patient.
- Responsible for individual and family therapy sessions for the purpose of psychoeducation and safety planning.
- Facilitates psychoeducation and psychotherapy groups with focus on evidence based therapeutic practices. This includes documentation of intervention and responses for each patient in the group.
- Ensures that releases, consents, and legal documents are signed in order.
- Interfaces with QM Coordinator and Nurses.
- Manages prudent utilization of financial resources.
- Utilizes quality improvement process in daily work so that improvement of systems in ongoing.
- Functions as liaison with outside agencies, leading network meetings, communicating treatment progress and discharge instructions to individuals or organizations responsible for continuing care of patients.
- Identifies cultural and religious needs, language barriers, etc., and modifies plan of care as necessary to accommodate these needs.
- Serves as an advocate for patients, family and significant other in regards to decisions regarding plan of care. Verbalizes the process for resolution to ethical dilemmas.
- Serves as professional and clinician role model to staff in interactions with patients, families, and staff.
- May be responsible for covering outpatient manager’s responsibilities if manager is unavailable.
- Provides education to the patient and/or when appropriate, his/her significant other(s) that can enhance their knowledge, skills, and those behaviors necessary to fully benefit from the healthcare interventions provided by the organization.
- Master’s degree in social work (MSW) required
- Licensed with New Hampshire Board of Mental Health Practice at time of hire. (LICSW or equivalent)
- 1-3 years of Social Work experience required
- BLS and CPI certifications required within 30 days of start date
