- Portneuf Medical Center is a nationally recognized healthcare leader with 205 beds, serving as the region's tertiary care and Level II Trauma Center, housing the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue
Responsibilities
Serves as the primary point of contact for coordinating all transfers in and out of the facility. Facilitates contact between providers to ensure smooth and efficient consultations and transfers. With understanding of local policies, procedures and resources, assist callers with logistical issues related to transfer of patients.
- Provides excellent customer service to internal and external providers and staff.
- Act as primary contact for referring and receiving physicians and facilities.
- Facilitates the smooth flow of patients through the facility including consults, admissions, and transfers into and out of PMC.
- Participates as a contributory member of the interdisciplinary team.
- Completes all documentation timely and accurately within departmental and facility requirements and timelines.
- Assumes responsibility for meeting in-service requirements and ongoing quality improvement activities.
Qualifications
Job Requirements:
- High school diploma or equivelant
- Previous hospital or clinical experience and knowledge of medical terminology
Preferred Requirements:
- Clinical professional licensure (EMS, CNA, LPN, etc.) highly preferred.
