JOB DUTIES:
- Conducts mental health and addictions initial diagnostic evaluations for children, adolescents, and adults.
- Completes CANS/ANSA, develops initial treatment plan and refers to appropriate level of care.
- Makes informed decisions about the treatment recommendations that will best meet the client's needs and schedules with that program(s) for follow up appointments. Communicates as needed with the program that the client is being referred to so that services can be started as quickly as possible.
- Develops and maintains positive working relationships with internal and external providers including clinicians, front desk schedulers, client financial services, and community partners.
CORE RESPONSIBILITIES:
- Completes all documentation as required by the agency's policies and procedures.
- Participates in weekly staffing with Psychiatrist, and weekly peer consultation with Outpatient Mental Health and Addictions colleagues.
- Requires travel between the Valparaiso, Portage, and Knox offices, and may include travel to Porter and/or Starke County Jail.
- Performs other duties as assigned.
EDUCATION & EXPERIENCE:
- Master's degree from an accredited college or university in Clinical Social Work, Clinical Mental Health Counseling, and/or Marriage and Family Therapy required.
- At least 3 years experience in providing mental health and addictions assessments and/or treatment, including provision of individual, group and family therapy required.
- Current LCSW, LMFT, LMHC licensure preferred; LSW, LMHC-A, LMFT-A required.
- Current state licensure in addictions preferred (LAC, LCAC).
