Candidates should be willing to travel throughout the Northern region of Michigan. Ideal candidates can provide 100-120 hours per month.

What You'll Do
- Conduct in-home health risk assessments for members in your geographic area
- Evaluate past medical history, review systems, reconcile medications, and perform vital signs and physical exams
- Use evidence-based practice screening, including point-of-care testing as needed
- Identify diagnoses for care management by Porter Care Guides
- Actively manage medical treatment based on assessment findings
- Formulate lists of current and past medical conditions using clinical knowledge and judgment
What You'll Need
- Valid unencumbered Michigan NP license
- 3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred)
- A strong sense of empathy, compassion, and friendliness
- Exceptional communication skills, both written and oral, with the ability to assist others (patients and coworkers alike) with sincerity, respect, and compassion

Pay: $100.00 - $110.00 per hour
