Job Summary
Currently seeking a full-time Case Manager to assist clients with linkage to community resources. This position will require use of personal vehicle and will provide mileage reimbursement.
Requirements:
- Associates Degree (or higher)
- 3 years of Behavioral Health Experience in lieu of degree (High school diploma required)
- Reliable transportation
Responsibilities:
- Conduct assessments of clients’ needs, strengths, and challenges to develop individualized care plans.
- Coordinate and manage services, including medical care, housing, and employment, ensuring that clients have access to the appropriate resources.
- Provide ongoing support to clients, monitor their progress, and adjust care plans as necessary.
- Collaborate with healthcare providers, social workers, and other community organizations to ensure comprehensive care for clients.
- Maintain accurate case files and documentation, ensuring compliance with privacy and confidentiality regulations.
- Advocate for clients, ensuring that they are receiving appropriate services and support.
- Assist clients in navigating complex systems, such as healthcare, social services, or legal systems.
