What are the responsibilities and job description for the Vocational Rehabilitation Case Manager position at Port Gamble S'Klallam Tribe?
The Vocational Rehabilitation Case Manager will work with the Career and Education Department and the Vulnerable Adult Program to identify and work with individuals who qualify for vocational rehabilitation services. This position will perform required case management services, maintaining client caseloads consisting of individual with varying degrees of disabling conditions to assist them in achieving a competitive employment outcome. The Case Manager will coordinate care in a client-centered manner, focusing on individual strengths, and the needs of the client, and conducting vocational evaluations to assist the client in job placement. Will work with other service providers to ensure wrap around services are provided and with future employers and other outside partner agencies.
Duties Include:
- Provides vocational rehabilitation counseling and devises work methods to provide vocational rehabilitation services to the client caseload.
- Determines client eligibility for Vocational Rehabilitation services by obtaining and analyzing relevant medical information identifying the disability-related barriers to employment that require Vocational Rehabilitation services, and determining if the disability is too significant to achieve an employment outcome.
- Provides vocational rehabilitation counseling to support, guide and foster clients insight and understanding of their strengths, capabilities and disability related barriers to employment.
- Coordinates with State Vocational Rehabilitation Programs and school districts.
- Promote the client’s self-reliance and informed choice, and assist in identifying the steps and Vocational Rehabilitation services required to achieve an employment outcome.
- Conducts a comprehensive assessment of each client’s needs for Vocational Rehabilitation services including: 1) their strengths, resources, priorities, concerns, abilities, capabilities and interests. 2) their disability-related barriers to employment, and 3) the suitability of their desired employment goal.
- Assists the client and the employer with post-employment services and provides the employer with necessary information regarding reasonable accommodations.
- Approves each clients individualized plan for employment, and revises or amends the plan when needed.
- Facilitates transportation needed to appointments, and services.
- Other duties as assigned and appropriate for this position.
Qualifications:
- Education: Bachelor’s degree required, but would consider a candidate actively enrolled in a Bachelor’s degree program.
- Experience: Minimum of two years’ experience working in a vocational rehabilitation role.
- License: Valid Washington State Drivers License
- Background Check: Able to pass a criminal background fingerprint check and drug screen.
Requirements:
- Basic personal computer skills are required, as well as the ability to learn word-processing, spreadsheet and other software programs.
- Filing, typing, and record keeping as needed
Physical Requirements:
- Regularly required to finger, handle, reach with hands and arms
- Frequently required to walk, sit, stand
- Occasionally required to lift and/or move up to 25 pounds
- Normal audio and visual acuity
Travel Requirements:
- Local travel required, Statewide and out of state travel may be required.
This position is not eligible for remote work
