Job purpose
The Program Coordinator (PC) role is vital to assuring a high standard of care and assurance for families regarding the quality of care provided by Poppy’s Promise. It includes developing positive rapport with people with disabilities and families/guardians, developing plans that focus on helping people achieve their highest level of learning and independence, monitoring progress to assure people have opportunities to meet their personal goals and guiding Direct Support Professionals (DSPs) to provide services that focus on skills in the areas of daily living, socialization, and problem-solving. This position will serve as a supervisor and role model for DSPs.

Duties and responsibilities
- Serve as the primary contact for people receiving services, families and professionals
- Assist people and their families to find and connect to resources within the community
- Facilitate team meetings regarding services provided to people
- Conduct assessments to determine individual interests, needs, risks, and dreams
- Develop individual plans that focus on person-centered supports and services to assist people to reach their goals
- Work with individual teams to develop creative ways for goals to be met through innovative teaching and support methods
- Monitor and track progress toward individual goals of those receiving services and initiate team discussion if progress is not being made or teaching methods are ineffective
- Assure DSPs are thoroughly trained on individual plans for people receiving services
- Provide supervision to DSPs and address human resources issues, to include performance evaluation and disciplinary action (if needed)
- Act as a role model for DSPs and colleagues by engaging with others in a professional, respectful, and positive manner
Qualifications:
Qualifications
Qualifications include:
- Possess a bachelor’s degree in social work, psychology, nursing, sociology, counseling, human development, special education, child development and family science, human resource management (human service track), criminal justice, occupational therapy, communication science/disorders or vocational rehabilitation. Other Human Services-related degrees may be considered.
- Have a valid driver’s license and proof of vehicle insurance.
- Be able to complete and pass all criminal and personal background checks as well as random drug screenings.
- Demonstrate the ability to communicate verbally and in writing in order to:
- Present information to all team members and outside professionals in a clear, concise manner.
- Share information with people using their preferred mode of communication.
- Adapt information to the various learning styles of DSPs.
- Demonstrate the ability to conduct both formal and informal assessment for the purposes of developing meaningful individualized support plans.
- Possess the ability to problem-solve issues that may arise for families, outside professionals, and DSPs.
- Be able to stand for extended periods of time and lift up to 50 pounds.
- Be able to respond to situations where people may attempt to run to or from an area (i.e. a small child running out into the street)
Working conditions
- This position will work primarily in an office environment Monday-Friday. Work duties may involve spending time in homes tor assessment and meeting purposes, as well as occasionally in community settings.
