The person in this position functions in the role of expert clinician and consultant for the care and treatment of a specified population of individuals with severe to profound physical and mental disabilities. In collaboration with the Director of Medical Services, Program Managers, program directors, therapists, physicians, parents / guardians, and service coordinators, the Clinical Nurse monitors the care of Individuals and provides support necessary to improve care and individual outcomes. The Clinical Nurse is a leader, patient advocate, agent of change, cost-effective practitioner, and a supervisor of Level 1
Medication Aide (MA1) staff.
The Clinical Nurse position is a position of great responsibility. This position provides care for those individuals that are the most vulnerable and requires a 100% of commitment to their health and well-being over the course of life
Specific Responsibilities
- May assist with In-Service training, especially the training and/or re-certification of MA1's, and other medical procedures in community living and community support services.
- Any other duties assigned by the Director of Medical Services and/or Vice President of Health Services.
- Flexible hours, including possible evenings and/or weekends as needed to serve the needs of the agency.
- Will be required to go into the community for meetings, to perform assessments, support individuals, and train staff.
- Provide RN oversight of LPN Clinical Nurse and LPN Staff Nurse as required by Missouri Nurse Practice Act, including but not limited to: training and review of all nursing tasks delegated to LPN, MA1 Charge staff, and Direct Support Professional (DSP) staff; Community RN – review and oversight care of LPN Clinical Nurse caseload as assigned to the RN
- Provide assessments for assigned caseload, and/or review of LPN Clinical Nurse caseload, which includes face to face evaluations with residents, and updates from direct care staff.
- Review, interpret, and analyze lab results of assigned caseload, and/or review of LPN Clinical Nurse caseload. Ensure that PBI's protocol for necessary labs and follow-ups is enforced.
- Review of medication administration records for assigned caseload, and/or review of LPN Clinical Nurse caseload for accuracy and MA1 Charge staff compliance, medication labels, monitor for side effects, effectiveness, frequency of PRN use, drug storage of routine and PRN medications, and check for supporting diagnosis.
- Review of records for assigned caseload, and/or review of LPN Clinical Nurse caseload to include weight, immunizations records, bowels, vitals, blood sugar, dietary, fluids, seizure, menses, skin, range of motion, consultation reports, event reports for falls, injuries, medication errors, hospitalizations and ER reports, and significant change in behavior, etc.
Job Qualifications
- Must be a licensed LPN or RN in the state of Missouri.
- Minimum of two (2) years of nursing experience preferred.
- Valid Missouri Driver's License in good standing.
- Experience and knowledge of care principles affecting individuals with severe to profound developmental disabilities.
- Demonstrated supervisory and organizational skills.
- Excellent interpersonal and communication skills (ability to communicate both orally and in writing.
- Excellent keyboard and software skills to keep computerized records.
- Ability to maintain confidential information.
- Knowledge of the care planning process.
- Proven ability to be flexible ad responsive to consumer needs.
- Able to perform consumer transfers using assistive lifting devices.
- Must be able to take and give accurate verbal directions and telephone orders.
- Must be CPR certified.
- Willing to obtain certification to teach MA1 class.
- Ability to work independently.
- Must have reliable, consistent transportation to the work site.
On-Call Management Standards and Practices
- On-call rotation is between all Clinical Nurses.
- On-call requires 24/7 coverage for a 7-day period in the rotation to be determined by the Director of Medical Services (typically every 5 weeks based on nurse staffing levels).
- On-call supports the medical needs of the residence after hours in the absence of an on-site Nurse.
- On-Call holiday annual rotation as determined by Director of Medical Services.
