We are an innovative organization, We are seeking experienced and compassionate Home Infusion Nurses to join our dynamic healthcare team at Ponos. In this role, you will play a crucial part in delivering specialized care to patients in the comfort of their homes, contributing to our mission of transforming healthcare and improving the quality of life for individuals dealing with acute pain. This role reports directly to the Nurse Manager.
RESPONSIBILITIES:
- Work in clinic setting or patient homes 5 days a week 8:30-5pm
- Administer home infusions for acute pain episodes, hydration needs, and scheduled biologic IV medications for patients with Sickle Cell Disease and gastrointestinal conditions, including but not limited to Crohn's Disease and related disorders.
- Certified for administration of biologic medications.
- Conduct thorough assessments of patients' health status and infusion needs.
- Collaborate with the multidisciplinary team to ensure coordinated and holistic patient care.
- Monitor patients during infusions, addressing any adverse reactions promptly.
- Educate patients and their families on infusion procedures and self-care.
- Document all aspects of patient care accurately and in a timely manner.
- Ensure compliance with safety protocols and infection control measures.
- Participate in care coordination efforts as a member of our team of licensed clinical social workers, registered dietitians, pharmacists, and registered nurses to ensure holistic patient care.
- Collaborate with healthcare team, to connect care and to develop personalized care plans.
- Implement preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed.
- Offer counseling and health education to patients and families as an advocate for our patients' holistic care and wellbeing.
QUALIFICATIONS:
- Bachelor’s degree in Nursing or equivalent
- Current RN license in the state of Virginia, CPR, BLS required, advanced certifications preferred (CRNI, ONCC, ACLS).
- Ability to work legally in the United States and Virginia.
- Active drivers license, free of restrictions.
- Strong commitment to health equity and culturally sensitive care.
- Willingness to travel and work in patient home environment within a 20 mile radius of Newport News, Norfolk and surrounding areas.
- 2 years proficiency in administering IV medications and managing infusion equipment.
- Excellent oral/written communication and interpersonal skills for effective patient education and collaboration within a multidisciplinary healthcare team.
- Must have the ability to work independently as well.
- Ability to apply logical thinking and problem-solving skills.
- Experience at an early-stage startup business a plus. Ability to readily “switch gears” as needed and in response to unexpected events and circumstances.

-This is a traveling position, RNs dispatch within a 30 mile radius daily, can anticipate 3-4 scheduled visits a day, in our member's homes blended with telehealth appointments.
-The working hours for this position are 8am-5pm, M-F and occasional weekend appointments as needed.
-This role does require an active RN in the state of VA, w/ Drivers license free of restrictions.
COMPENSATION/ SALARY:
· Competitive salary with performance-based incentives.
· The targeted base salary compensation range for this position is $105,000-$110,000 for Full Time. PRN position are now open and interviewing, apply today to meet with our hiring team!
