Job Summary
We are seeking a dedicated and compassionate Wound Care Nurse to join our healthcare team. The ideal candidate will have a strong background in wound management and nursing care, particularly for patients in geriatrics and senior care settings. This role involves assessing, planning, implementing, and evaluating wound care treatments to promote healing and prevent complications. The Wound Care Nurse will work closely with patients, families, and interdisciplinary teams to ensure the highest quality of care.
Duties
- Conduct thorough assessments of patients' wounds and overall health status.
- Develop individualized care plans based on best practices and patient needs.
- Perform wound care procedures, including dressing changes, debridement, and infection control measures.
- Educate patients and their families on wound care techniques and prevention strategies.
- Collaborate with healthcare providers to coordinate comprehensive patient care.
- Maintain accurate documentation of patient assessments, interventions, and outcomes using Cerner or similar electronic health record systems.
- Monitor patients for signs of infection or complications related to wounds.
- Provide support in urgent care situations as needed.
Experience
- Previous experience in geriatrics, senior care, or memory care is highly desirable.
- Proficiency in venipuncture techniques is preferred.
- Familiarity with durable medical equipment (DME) relevant to wound care management is a plus.
- Knowledge of infection control practices and internal medicine principles is essential.
- Experience working in acute or long-term care settings will be beneficial.
