Responsibilities:
- Assess patient needs and develop individualized care plans to support health and wellness goals
- Coordinate and monitor healthcare services, referrals, and community resources for assigned caseload
- Serve as a primary point of contact and advocate for patients, families, and caregivers
- Collaborate with multidisciplinary care teams including physicians, social workers, and specialists
- Maintain accurate and up-to-date case documentation in compliance with regulatory standards
- Conduct follow-up assessments to evaluate patient progress and adjust care plans as needed
- Facilitate transitions of care and discharge planning to ensure continuity of services
Requirements:
- Bachelor's degree in Social Work, Nursing, Healthcare Administration, Medical Assistant, or a related field
- 1-3 years of experience in case management, care coordination, or a similar healthcare role
- Knowledge of community resources, healthcare systems, and social determinants of health
- Strong communication, organizational, and problem-solving skills
- Proficiency with electronic health records (EHR) and case management software
- CCM (Certified Case Manager) certification is a plus
- Empathetic, patient-focused demeanor with the ability to work with diverse populations
