What are the responsibilities and job description for the Case Manager position at PNB?
PNB is looking for a self-motivated, reliable individual with excellent communication and is personable, with good integrity, ethics/ moral compass and a compassion to make a difference in the lives of others. We need applicants based in Raleigh, Mercer, Fayette, Nicholas and Wayne County. (PNB covers multiple Southern WV counties). This is NOT a job you can work part-time or as a second job. You MUST be available Monday through Friday 8 am - 4 pm.
The WV Medicaid Intellectual and Developmental Disability Waiver Program Case Management establishes, along with the individuals served, a life-long, person-centered, goal-oriented process for coordinating the supports (both natural and paid), range of services, instruction and assistance needed by persons with developmental disabilities.
Must be able to meet Developmental Disabilities Professionals (DDP) qualifications, demonstrate excellent verbal and written communication skills, and be willing to work flexible hours. Acceptable degrees that meet DDP qualifications include: bachelor’s degree (4 year degree) in education, social work or in human services. Training will be provided. The position is contract and does not include benefits.
