We’re looking for an experienced Registered Nurse to coordinate care for patients with chronic conditions in our clinic setting. You’ll help track gaps in care, arrange follow-ups, support patient education, and collaborate with primary and specialty providers to improve health outcomes.
Compensation
- Hourly Rate: $46.18 – $65.84 (based on clinical pay scale)
Job Status / Shift Information
- Full-time, benefited
- Standard clinic daytime schedule
Qualifications
- Active California RN License
- Graduate of an accredited Nursing program
- Current CPR certification
- 3 years in acute care, urgent care, or outpatient/family practice
- Strong EMR skills ability to organize, triage, and communicate clearly
