- Support: Enable residents and families to navigate the emotional and practical impact of illness.
- Advocacy: Arrange for needs to be met through appropriate referrals to outside agencies and professional services.
- Management: Participate in resident care management and ensure all social service provisions are met.
- Connection: Act as a primary resource for residents, families, and significant others.
