Duties
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
- Monitor and observe patient health status by recording vital signs such as blood pressure, pulse, temperature, and respiration rates
- Support medication administration under supervision or according to care plans while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards
- Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral support
- Operate electronic health record (EHR) systems to document patient progress accurately and maintain detailed medical records
- Assist with transferring patients safely using patient transfer methods such as Hoyer lifts or gait belts; perform patient transfers with proper technique to prevent injury
- Support end-of-life care by offering emotional comfort and ensuring dignity for terminally ill patients in hospice settings
Requirements
- Proven experience working as a Home Health Aide or in assisted living facilities; experience with geriatrics and long-term care preferred
- Knowledge of medical terminology, catheter care, wound care, and basic life support (BLS) certification including CPR training
- Ability to perform heavy lifting safely and handle physically demanding tasks such as laundry, cleaning, meal preparation, and housekeeping duties
- Familiarity with EMR (Electronic Medical Records) systems and medical documentation practices
- Strong communication skills for effective client interaction and collaboration with healthcare teams
- Valid certification as a Home Health Aide or equivalent credential; additional certifications such as first aid are advantageous
- Experience working with individuals with disabilities or developmental challenges is highly valued
- Knowledge of home health care regulations and compliance standards to ensure quality service delivery
