Lead for Health Home/Care Management Agency

Job Summary 

Platinum Healthcare, a care management agency for New York State’s Medicaid Health Home program, is seeking a Director of Health Home Program to oversee operations in the Bronx and Manhattan regions, primarily based out of our Midtown Manhattan headquarters. 

The lead will lead and expand an already established team of Care Coordinators, providing operational leadership, program oversight, and ongoing staff development. This role is ideal for a candidate with strong Health Home experience who can both manage systems and support frontline staff. 

While primarily office-based, the Director will regularly be out in the field, meeting with community organizations, conducting field-based supervision, supporting care coordinators during complex cases, and strengthening community relationships. 

Responsibilities 

• Provide overall leadership and day-to-day management of the Health Home Program in the Bronx and Manhattan regions. 

• Oversee and support the existing Care Coordinator team, ensuring strong performance, growth, and high-quality member engagement. 

• Supervise program staff including training, coaching, and handling patient enrollments. 

• Ensure care coordination activities (assessments, care plans, linkages, hospital transitions, outreach, crisis intervention, etc.) meet NYS Health Home standards. 

• Monitor program performance metrics such as utilization, engagement, quality indicators, and documentation timeliness & implement quality improvement strategies as needed. 

• Offer clinical and administrative guidance to staff regarding complex cases, social determinants of health, and access to entitlements (SSI/SSD, SNAP, HEAP, housing, transportation, etc.). 

• Conduct regular field-based supervision and support, joining staff in the community when providing face-to-face outreach, engagement and service planning. 

• Maintain compliance with NYS Medicaid Health Home regulations and all agency policies. 

• Collaborate with hospitals, behavioral health providers, medical providers, and community organizations to ensure coordinated transitions of care. 

• Manage program workflows, caseload allocation, staffing coverage, and operational efficiency. 

• Contribute to program growth, planning, and strategic initiatives. 

• Represent Platinum Healthcare at internal and external meetings relevant to the Health Home program. 

Qualifications 

• Health Home experience is required (experience in a CMA, Lead HH, or directly with NYS Health Home program). 

• Master’s degree in Social Work, Mental Health Counseling, Psychology, Public Health, Health Administration, or related field preferred or Bachelor’s degree with significant experience considered. 

• Minimum 3–5 years of care management or behavioral health experience, with at least 1-2 years in a supervisor or leadership role. 

• Experience serving adults with serious mental illness, chronic medical conditions, and/or substance use disorders. 

• Strong understanding of NYS Medicaid, Health Home regulations, billing, and entitlements. Working knowledge of HH/CMA related technologies such as Foothold Care Management, ePaces, MAPPS 

• Proven leadership skills with the ability to support, mentor, and strengthen a care coordination team. 

• Excellent communication, organizational, documentation, and problem-solving skills. 

• Ability and willingness to travel throughout the Bronx and Manhattan, including field work and community visits. 

• Bilingual English/Spanish is a plus. 

Schedule & Pay 

• Full-time, Monday–Friday, 9 AM- 5 PM 

• Combination of office-based work and regular field presence. 

• Salary: $85,000–$90,000 annually
