Under the general supervision of the Director of Navigation Services and Patient Experience, the Abortion Patient Navigator serves as a vital advocate and coordinator for patients seeking abortion care. The Navigator coordinates all activities and resources for the abortion access program across multiple sites, managing administrative tasks including scheduling meetings with community health professionals, arranging travel, and processing expense reports. The Patient Navigator provides compassionate, in-person assistance tailored to each patient's unique needs, guiding them through the abortion care system and helping them navigate complex healthcare environments. A core responsibility is identifying and working to overcome barriers patients face in accessing necessary care and services. The Navigator must quickly and effectively synthesize patient needs and serve as a liaison between patients, medical professionals, and community resources to create individualized care plans. The Patient Navigator works collaboratively with fellow Patient Navigators, healthcare providers, center managers, and clinical support staff.
The Patient Navigator is a community health professional who provides field-based case management services to patients accessing abortion care, serving as their primary point of contact throughout the care journey. Central to this role is a commitment to removing barriers to care by identifying critical resources, helping patients navigate healthcare services and systems, and advocating for their overall health and wellbeing. The Patient Navigator works closely with the Abortion Access team—including clinicians, nurses, and other clinical staff—to coordinate comprehensive support and promote positive patient health outcomes.
ESSENTIAL FUNCTIONS Some of the responsibilities of the patient navigator may include: 1. Organize schedules and manage appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences. 2. Facilitate communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care. 3. Inform the patient on financial aspects of care and link the patient to financial resources as needed. 4. Works with the patient to identify logistical needs to access care and link the patient to appropriate resources as needed. 5. Works with the patient to identify other support services as needed and provide appropriate linkages to care. 6. Accommodates interpretive language services when needed. 7. Facilitates pre- and post-abortion visits and ensures continuity of care and linkage to community resources. 8. Collects, tracks, and reports required program data to appropriate stakeholders. 9. Maintains strict confidentiality in accordance with agency policies.
