Home Health RN Job Details

- Location: Greater Milwaukee area, Wisconsin (WI 53214)
- Start Date: September 15, 2025
- End Date: December 14, 2025
- Contract Length: 13 weeks
- Schedule: Monday – Friday, 8:00 AM – 5:00 PM
- Guaranteed Hours: 40 hours per week
- Shift Duration: 8-hour days
- Call Requirement: None
- EMR Systems: HCHB and OASIS
- Estimated Weekly Salary Range: $1950-$2000
- Active Registered Nurse (RN) license in Wisconsin
- At least 1 year of recent Home Health RN experience (within the past 3 years)
- Proficiency with HCHB and OASIS EMR systems
- Flexibility with territory assignments in the Greater Milwaukee area
- Excellent communication and organizational skills
- Deliver high-quality nursing care to patients in their homes across Milwaukee, WI
- Complete accurate and timely documentation using HCHB and OASIS
- Meet minimum productivity expectation of 30 points per week
- Collaborate with interdisciplinary healthcare teams for comprehensive patient care
- Adapt to changing territory assignments to best serve patient needs
