Home Health LPN Job Details
- Location: Dover, DE 19904 (Delaware home health jobs)
- Assignment Type: Travel LPN position
- Healthcare Setting: Home Health
- Contract Duration: 13 weeks (8/25/2025 – 11/24/2025)
- Schedule: Monday – Friday, 8:00 AM – 5:00 PM
- Shift: 8-hour day shifts
- Hours per Week: 40
- Estimated Weekly Salary Range: $1590–$1630
- Average Patients per Day: 5–8
- Territory: Up to 75-mile radius from home clinic; minimum 250 miles/week travel expected
- Mileage: Reimbursed at IRS standard rate (from patient home to patient home)
- Active LPN license in Delaware (must have license in hand)
- At least 1 year of recent adult home health or hospice nursing experience
- Current CPR/BLS certification
- Proficiency with HomeCare HomeBase (HCHB) EMR (recent use or willingness to retrain)
- Reliable vehicle, valid driver’s license, and auto insurance
- Willingness to travel up to 75 miles from assigned care center
- Ability to complete clinical documentation within 24 hours of patient visit
- Must accept all assigned patients regardless of travel distance between appointments
- Not currently employed directly by any Amedisys location
- Open to on-call rotations as needed
- Coordinate and deliver home nursing care in accordance with physician orders and agency policies
- Supervise and evaluate Home Health Aides and other LPNs/LVNs as assigned
- Conduct home visits for various cases, including start of care, wound care, and follow-up visits
- Ensure timely and accurate documentation of all patient care activities using EMR
- Participate in office meetings and maintain communication with the clinical manager
- Meet weekly productivity expectations (30 points/week, approximately 40 hours)
- Deliver compassionate, high-quality patient care while upholding professional nursing standards
