The Physical Therapist provides outpatient rehabilitation services primarily in-home for the older adult population, assessing and developing personalized therapy programs to promote recovery, independence, and improved quality of life.

The Physical Therapist provides outpatient rehabilitation services primarily in-home for the older adult population, assessing and developing personalized therapy programs to promote recovery, independence, and improved quality of life.
Key Responsibilities
- Evaluate patients through examinations, develop treatment plans, and implement therapeutic techniques such as therapeutic exercises, manual therapy, and modalities.
- Educate and train patients and caregivers to foster independence and health maintenance.
- Make referrals to other disciplines, coordinate with the rehabilitation team, and adapt plans based on patient needs.
- Supervise support personnel, ensure timely documentation, and maintain equipment and safety standards.
- Perform functional mobility assessments, fabricate and train in adaptive devices, and incorporate health and wellness programming.
- Valid Physical Therapy license in the practice state(s) or proof of eligibility.
- Master’s degree in Physical Therapy or related field, or Bachelor’s plus extensive experience.
- Reliable transportation and a valid driver’s license.
