Plan A is a nonprofit organization that provides free healthcare services and care coordination in the Mississippi Delta. This role is specifically part of the Healthy Families, Healthy Futures program focused on pregnant women and families with infants up to 18 months.
Position Summary
The Maternal Health Care Coordination Supervisor provides both direct care coordination services to program participants and oversight of a team of Care Coordinators. This vital role involves managing a team of care coordinators, ensuring seamless communication between patients, providers, and community resources, and promoting high-quality, patient-centered, timely support to enrolled families.
The Supervisor maintains a reduced caseload while supervising staff, monitoring performance, and ensuring adherence to grant requirements, program protocols, and documentation standards. The ideal candidate will possess extensive experience in clinical coordination, a strong understanding of the needs of pregnant women and families, and exceptional leadership skills to foster a collaborative and impactful work environment.
Key Responsibilities
Supervision & Team Management
- Supervise and support a team of approximately 5-6 Care Coordinators, ensuring adherence to clinical protocols and organizational standards.
- Conduct regular supervision meetings (individual and team-based)
- Coordinate multidisciplinary care plans for pregnant women, new mothers, and their families, integrating services such as prenatal education, postpartum support, and neonatal care.
- Support staff with complex participant cases
- Review and advise staff on outreach and enrollment efforts, in collaboration with the Healthy Start Project Manager
- Ensure appropriate resource referrals are made to participants
- Monitor staff productivity, caseloads, and service quality
- Review documentation for accuracy, timeliness, and compliance
Care Coordination Program Management
- Support data collection, reporting, and quality improvement initiatives
- Evaluate and provide subject matter input on maternal and child health training curricula
- Participate in audits, site visits, and grant reporting as needed
- Collaborate with supervisors to maintain strong referral partnerships
Qualifications
Required:
- Master’s degree in Social Work (MSW) or related field
- Minimum 4-6 years of experience in care coordination, case management, or maternal/child health
- Experience working with underserved or high-risk populations
- Strong organizational, documentation, and communication skills
- Resident of the Delta
- Able to travel within the community
- Supervisory or team leadership experience
- Able to lift and move boxes up to 25 lb (specifically for preparation of items to be distributed to parents such as diapers)
Preferred:
- Experience in maternal, perinatal, or infant health programs
Work Environment & Schedule
- Combination of work-from-home, office, community-based, and/or home visits
- Local travel
- Standard business hours with occasional flexibility based on participant needs
Compensation
Salary range: $55,000-65,000
