About the Role:
Provide direct patient care, including administering medications, dressing wounds, and performing vital signs
Develop and implement individualized care plans in collaboration with interdisciplinary team members
Monitor and document patient progress, reporting any changes or concerns to the nursing team
Maintain accurate and up-to-date patient records and charts
Collaborate with other healthcare professionals to ensure seamless patient care
Responsibilities:
Deliver high-quality care to patients with a variety of medical and behavioral health conditions
Perform basic medical procedures, including injections, wound care, and electrolyte testing
Educate patients and families on disease management, medication administration, and self-care techniques
Contribute to the development and implementation of individualized care plans
Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements:
Current LVN licensure in the state of Texas
Prior experience as an LVN in a healthcare setting
Strong communication and interpersonal skills
