- Assist the MDS Coordinator with completion and tracking of MDS assessments and documentation
- Monitor assessment schedules and ensure timely completion of OBRA, Medicare, and insurance-required assessments
- Maintain accurate resident records and audit documentation for completeness and compliance
- Coordinate with nursing, therapy, dietary, social services, and other interdisciplinary departments regarding resident assessments
- Enter and update resident information within electronic medical record (EMR) systems
- Assist with care plan preparation and interdisciplinary care plan meetings
- Monitor physician documentation and supporting clinical records for accuracy
- Help ensure compliance with federal, state, and company regulations related to MDS and reimbursement processes
- Assist with Medicare and managed care documentation tracking as needed
- Maintain confidentiality of resident and facility information in accordance with HIPAA guidelines
- Support the nursing department with administrative duties and special projects as assigned
- Active LPN or RN license in the state of Indiana or multi state nursing license
- Previous skilled nursing or long-term care experience preferred
- Prior MDS, Medicare, or medical records experience preferred
- Knowledge of PDPM, OBRA, and Medicare documentation processes preferred
- Strong organizational skills and attention to detail
- Excellent communication and teamwork abilities
- Proficiency with electronic medical records and Microsoft Office applications preferred
- Ability to multitask and work efficiently in a fast-paced healthcare environment
