Role Description

This is a full-time hybrid role based in Phoenix, AZ, with some work-from-home flexibility. As a Home Health Care Specialist, you will deliver high-quality therapy services, including occupational, physical, and speech therapy, to clients with developmental disabilities. You will work closely with clients and their families to assess needs, develop tailored care plans, and implement therapy in their natural environments such as homes, schools, and community settings. Collaboration with other professionals and maintaining accurate documentation will also be integral to this role.

Qualifications

• Proficiency in providing occupational, physical, or speech therapy services
• Strong skills in client assessment, personalized care planning, and progress monitoring
• Excellent communication, interpersonal, and relationship-building abilities with clients and families
• Attention to detail and aptitude for accurate documentation and record-keeping
• Team-oriented mindset with a strong sense of professionalism and adaptability
• Relevant education and licensure as an occupational, physical, or speech therapist
• Experience working with individuals with developmental disabilities is a plus
• Ability to handle a hybrid work schedule effectively, including travel within Phoenix, AZ for on-site client visits
